it doesn't work if you've got alt already bound to a hotkey, for example if you have two hotkeys for lich's ice armour, W to manual cast and alt-W to toggle autocast, pressing alt-W won't self cast.
same with items. I think a lot of people (including myself) use quickcast as their primary item hotkeys and use alt for regular cast. I just take off regular cast and only use quick cast, since most of the time the only reason I do use regular cast is to cast things on myself.
