It's a problem with the API. It occurs when a player doesn't use their skill point at level 5 then skills up twice at level 6 (in this case, Q then E). The API does not properly represent the situation:

except he didn't hack. this bug happens usually if people level up twice, and skill their ult with what is considered their "level 5 point". i watched the replay and bloodseeker did this, went from 4->6, levelling ulti then a third point in thirst
